Mandated Reporting Procedures

  1. Call 1-800-540-4000 to report
  2. Get the 19 digit referral number. You will need this number when you log on.
  3. Log onto https://mandreptla.org/ and click on "Create Suspected Child Abuse Report". (It may take about 2 hours before you can log on using the 19 digit number from the time you call)
  4. Print a copy for your record
