California Healthy Kids Survey (CHKS)
2023-2024 CHKS SURVEY for 5th and 7th Grade Students
East Whittier City School District is committed to creating and maintaining a safe environment for your child to learn. We need your help in gathering valuable information related to health, school safety, the learning environment as well as alcohol, tobacco, and other drug issues. This information helps our district create better programs to help our students be happy, healthy, and safe at school. The information gathered from previous surveys has been used to obtain funding to expand and improve our physical education, counseling, and after-school programs. The information is also used in developing our District’s local control accountability plan (LCAP).
Participation in the survey is voluntary and we hope you will give permission so that your child's opinion and knowledge can be part of the information we receive to make our schools better. We want to continue meeting the ongoing and changing needs of our students. The important information they provide us through this survey will help us do that.
The survey is anonymous. No names or any other identifying information is connected to the answers, except for the name of the school. To allow your child to participate please follow the following steps according to your child’s grade level.
Elementary 5th Grade Parents: To allow your child to participate, please complete the permission slip provided by your child's teacher and return to your child’s school. We need your permission for 5th-grade students to take the survey. You can also print a copy of the consent form below.
Middle School 7th Grade Parents: Please note that the CHKS survey for middle school students requires passive consent, which means you only complete the form if you do NOT want your child to participate. Please submit this notification to your child's school by March 1, 2024.
